2008 Other Benefits

Home health services (50 days per year)	20% and all charges over $135 per day maximum (no deductible)	20% and all charges over $135 per day maximum (no deductible)

Chiropractic Care (initial office visit)	$20	30% after $300 deductible

Enhanced Eldercare Services - This program provides expert assistance from a Care Advocate to members and spouses that care for an elderly relative or disabled dependent. This benefit gives you a bank of six free hours per calendar year, which can be used for a combination of services. To use Enhanced Eldercare Services, simply call 1-877-468-1016.
