2008 Other Benefits

Physical and occupational therapy (a combined total of 50 visits per calendar year)

  • 15% of Plan Allowance(1)
  • 30% of the Plan Allowance, and the difference, if any, between our allowance and the billed amount (2)

Speech therapy (up to 30 visits per calendar year)

  • 15% of Plan Allowance (1)
  • 30% of the Plan Allowance, and the difference, if any, between our allowance and the billed amount (2)
Home health services (50 days per year)
  • 20% and all charges over $135 per day maximum (no deductible)
  • 20% and all charges over $135 per day maximum (no deductible)
Laboratory Services
  • nothing using Quest Diagnostics
30% of the Plan Allowance, and the difference, if any, between our allowance and the billed amount (2)
Chiropractic Care (initial set of spinal x-rays and 12 spinal manipulations per year)
  • 15% of the negotiated rate (1)
30% of the Plan Allowance, and the difference, if any, between our allowance and the billed amount (2)
Chiropractic Care (initial office visit) $20 30% after $300 deductible

(1) Subject to the $250 PPO calendar year deductible.
(2) Subject to the $300 Non-PPO calendar year deductible.

Enhanced Eldercare Services - This program provides expert assistance from a Care Advocate to members and spouses that care for an elderly relative or disabled dependent. This benefit gives you a bank of six free hours per calendar year, which can be used for a combination of services. To use Enhanced Eldercare Services, simply call 1-877-468-1016.
